Call of Duty World at War on Veteran difficulty. The entire campaign is really hard, but about halfway through the 2nd to last level, things get ridiculous. The last level of CoD 4 campaign on Veteran might be even harder though, from what I've heard.

I have to say, I'm playing Far Cry 1 on Normal difficulty, and it's really difficult/challenging. I think there were at least 2 harder difficulty settings, so I can't even imagine what those must be like.

Hoshigami: Ruining Blue Earth.

Had it given to me as a gift, and I really, really tried to play and enjoy the game, but even after hours and hours of grinding up character levels proved to be pointless and I moved on to hours and hours of grinding up spell levels, hoping that was the secret to the game, the steep difficulty curve and permanent character death, even seeming plot-important characters, I eventually gave up.
